Chronic or Long Term Conditions are those that cannot, at present, be cured, but people living with these conditions can be supported to maintain a good quality of life.
Many people might be living with more than one long term condition.
We work with our patients to help them better manage chronic conditions such as:
Project Food Cooking with Diabetes courses
Following the extremely successful delivery of their previous Cooking with Diabetes course, we’re delighted to learn that Project Foodwill be starting their next FREE course soon.
Cooking with Diabetes is a supportive, practical cooking course for people living with or affected by pre-diabetes or Type 2 diabetes. Please note that this course is no longer a drop‑in – places must be booked in advance. This course is ideal for individuals who would benefit from building confidence in the kitchen, learning how to prepare balanced meals, and developing sustainable, healthy eating habits in a supportive environment.
Topics will include:
- Understanding Type 2 Diabetes & Healthy Cooking Basics
- Building Balanced Meals
- Low‑Carbohydrate & Heart‑Healthy Cooking
- Meal Planning & Batch Cooking
- Creating Long‑Term Healthy Habits

Non-medical support
While your GP will continue to support your ongoing health conditions, often taking care of your health involves more than just medicine.
Our Care Coordinators focus on personalised care and concentrate on what matters to you by getting to know you and your personal situation. A Care Coordinator can support you in navigating the health system and accessing additional local support and services.
This service is for you if:
- You are over 18
- You need help finding your way around different health, social care and support services
- You have one or more long term conditions
Social Prescribing Resources
Social Prescribers from the Jurassic Coast Primary Care Network (PCN) work alongside us to provide extra time, a listening ear, and practical support to help you live a healthier, happier life.
The Social Prescribing team have collated a library of trusted local and national services related to Long Term Conditions.
