Your request will be sent to the PPG Secretary, and they will add your details to the PPG contact list.

How your information is used

Your name, email address, GP practice and application details will be shared with the PPG Secretary and held on a secure central PPG database, in line with NHS Dorset data protection requirements.

Your details will be used to:
  • Send PPG surveys and requests for feedback
  • Share news and updates about your practice
  • Send meeting invitations and minutes

Your information will not be used for marketing or shared with a third party.
